Dear CGU Students,
All CGU students must do one of the following: submit proof of vaccination, submit an exemption request or submit an affidavit declaring no return to campus is required.
- Student Health Services Medical Record Upload Portal
- Student Health Services medical or religious exemption form
- Affidavit due to Remote Work / Study
Students that do not act will be un-enrolled and are not approved to attend in-person classes, activities, or events. It is important that every member of our community inform CGU of their status, so that we can determine vaccination rates and manage the safety of our campus.
Students that comply with vaccination or exemption requirements will be approved by CGU to access campus buildings and attend in-person classes and events, including outside events. All approved students will be notified before August 23.
Students that have completed the Affidavit due to Remote Study / Work are not approved to attend in-person classes or events and will be un-enrolled in their in-person classes prior to the beginning of the term.
Before coming to campus, be sure you understand all procedures and requirements. Keep the daily health check symptom form in a convenient place for you to access and complete before each campus visit. If you feel any cold- or flu-like symptoms, do not come to campus.
To help us provide timely information to you, update your People Soft contact and emergency contact information prior to the start of the fall 2021 academic year to reflect your current mailing, permanent address, phone number and non-cgu email.
